PCF CAHPS

Primary Care First is a set of voluntary alternative five-year payment options that reward value and quality by offering an innovative payment model in efforts to support advanced primary care. Primary care clinicians provided input based on the original principles with Comprehensive Primary Care Plus (CPC+) that focus on: prioritizing clinician-patient relationships; enhancing care for patients with complex chronic needs, and focusing financial incentives on improved health outcomes. There are currently 822 practices participating across the 26 regions and include 14 payer partners within Cohort 1. The second Cohort will begin January 2022 and deadline to apply for participation was completed as of May 2021. It will be anticipated a third cohort could open for application in spring of 2022 for consideration. If you applied to be part of Cohort 2, selection will take place late summer or fall of 2021. CMS has not released any consideration for adding additional regions to the PCF model.
