Canopy Surveys HHCAHPS
HHCAHPS surveys, handled start to finish.
HHCAHPS (Home Health CAHPS) is the CMS-required patient experience survey for Medicare-certified home health agencies. Canopy administers HHCAHPS alongside the rest of your survey lines — one platform, one team, from sampling through reporting.
What is the HHCAHPS survey?
HHCAHPS — also written HH CAHPS, the Home Health Consumer Assessment of Healthcare Providers and Systems — is the standardized survey of home health patients. It asks how well the agency’s team communicated, how care was delivered, and how patients rate the agency overall. CMS requires it of Medicare-certified home health agencies and publicly reports the results.
In rural health systems, home health is usually hospital-based — the same organization already fielding HCAHPS on the inpatient side. Running HHCAHPS through the same platform means one vendor relationship, one team, and one set of dashboards for the whole system.
How the HHCAHPS line works.
Same rigor as your hospital lines, adapted to patients who are cared for at home.
-
1
We sample your patients
Eligible home health patients are drawn to HHCAHPS specification, month after month — including the small monthly counts rural agencies actually have.
-
2
We field the survey
Multi-mode outreach reaches patients at home — where home health patients actually are — so more of your patients are heard.
-
3
We report
Scores, comments, and trends land in the same dashboards as your hospital lines — the whole system's experience in one place.
Rural is not an afterthought here
Care that goes home deserves measuring too.
Rural home health agencies serve long distances and short censuses — and their survey program has to respect both. We’ve spent 11 years building survey operations around exactly these realities for rural hospitals and their agencies.
- Built for the small monthly censuses rural agencies actually have
- A named CAHPS specialist who answers the phone — not a ticket queue
- Home health and hospital lines together in one dashboard
- 11 years running survey programs for hospitals across the country

Yes — that's the point of one platform. HHCAHPS is administered alongside HCAHPS and the rest of your lines, with one team and one set of dashboards across the whole system.
As responses land. Scores and patient comments surface in your Canopy dashboards while the care episode is still recent — not months later when results are published.
Surveys are administered on a HIPAA-secure platform. This marketing website handles no patient data at all.
