Canopy Surveys
Every survey line. One trusted platform.
Canopy Surveys is patient satisfaction survey software and full survey administration in one. We sample, field, and submit the CMS-required surveys — HCAHPS and OAS CAHPS, both CMS-approved — and run the full family of patient experience survey lines through one platform, with one team.
Every purchase also includes six free months of Radia 360 and Concise. See what’s included
The required lines. The rest of the family. One roof.
Inpatient to hospice, clinic to swing bed — every patient experience survey your facility runs, fielded by one team, reported in one place, and — for the required lines — submitted to CMS for you, on schedule. (New to the survey family? Start with our guide: What is CAHPS?)

Which lines actually apply to you
The short version, by facility type.
HCAHPS — inpatient
Required for hospitals paid under Medicare's Inpatient Quality Reporting program. Critical Access Hospitals aren't paid that way, so it isn't required of them — many field it anyway, and report it through MBQIP's patient engagement measure.
OAS CAHPS — outpatient surgery
Required for hospital outpatient departments and ambulatory surgery centers in Medicare's outpatient quality reporting programs. Facilities outside those programs aren't bound by it.
Information Transfer PRO-PM — outpatient procedures
The newest requirement, in the same outpatient reporting program as OAS CAHPS: voluntary through 2026, required for hospital outpatient departments starting January 2027. Critical Access Hospitals are exempt.
Every other line
ED, clinic, home health, swing bed, hospice, nursing home, rounding, and ancillary surveys are never CMS-required. Hospitals run them because the department matters to them, not because a program says to.
Requirements follow the programs you participate in, so the answer is facility-specific.

All your surveys. One dashboard.
Every location gets its own dashboard with scores updating the moment responses land, comments left by patients, and national comparisons that show where you actually stand.
Biggest mover: Responsiveness of staff, up 9 points since Q1.
Every Canopy client benchmarks anonymously against national data. State-level comparison is available where enough data exists.
“Everything was explained before I went home, including what to watch for. That made a real difference.”
Export any view as a chart, table, or board-ready summary.
Results while you can still act.
Collecting responses is only the beginning — the value is in what your team does next.
Interactive dashboards
Scores and comments surface the day responses land.
Trends, not snapshots
Watch performance move across quarters and years.
Benchmarks that matter
See results against national comparisons, line by line.
Department views
Every unit sees its own results and its own priorities.
Executive summaries
Board-ready reporting without the report-building.
People who pick up the phone
CAHPS specialists run your onboarding and watch every milestone.
Live support
When you call, a person answers.
Canopy support is a live team — people who know your survey program, not a ticket queue. If you’ve run surveys with a national vendor, you know how rare that is. If you’re a small facility, you know it even better: the big vendors save their attention for the big systems. We don’t.
- A live team on the phone — not just a ticket portal
- Specialists who know your program, not a script
- A 25-bed hospital gets the same team as a health system
Why trust a “new” vendor
The platform isn’t new. The name on the door is.
For 11 years, our team has built and run this survey platform for hospitals across the country: the sampling, the fielding, the reporting, and the data behind every CMS submission. The only step that wasn’t ours was the final submission itself. With Canopy’s own CMS approvals, it is now — one team, one improved platform, end to end.
- 11 years building and running these exact survey programs
- 3.3 million+ surveys sent in the last year
- Same improved platform, now under the Canopy name
- CMS approvals held directly: HCAHPS and OAS CAHPS
- The full process handled — sampling through submission
You field nothing. You miss nothing.
You watch results land in your live dashboard — we do everything that puts them there, to exact CMS specification, every cycle.
-
1
We sample to protocol
Eligible patients are drawn to exact CMS specification — the part that quietly disqualifies data when it's done wrong.
-
2
We field the surveys
Multi-mode outreach meets patients where they are, so more voices land in your results.
-
3
We submit and report
Data goes to CMS on schedule; results reach you while you can still act on them — not in a quarterly PDF.
Beyond the required lines.
The same platform and team run custom programs for every voice in your organization:

Culture, morale, and retention risk — before people leave.
Physician engagement
Practice satisfaction, workflows, and alignment.
Community perception
How your market sees you — for planning and outreach.
Board & leadership
Governance assessments with objective feedback.

Frequently asked questions.
Yes. Canopy holds CMS approval for HCAHPS and OAS CAHPS, each approved directly to Canopy — you'll find us on the official HCAHPS approved vendor list and OAS CAHPS approved vendor list — and our team has been fielding these surveys for hospitals for 11 years.
HCAHPS, OAS CAHPS, ED Surveys, CG CAHPS, HHCAHPS, Hospice Surveys, Nursing Home Surveys, Swing Bed Surveys, Rounding, the Information Transfer PRO-PM, and Ancillary Services — all administered through one platform, with custom survey programs available beyond the standard lines.
Yes — end to end. We sample to protocol, field the surveys, and submit your data to CMS on schedule. Your team reviews results; we run the process.
Less than you'd think. We map the transition with you — protocols, timelines, and notifications — so no reporting cycle is missed. Most of the work is ours, not yours.
Yes. Beyond the CMS lines we build custom programs — employee engagement, physician engagement, community perception, board assessments, and one-off studies — designed around your goals rather than a generic template.
Surveys are administered on a HIPAA-secure platform. This marketing website handles no patient data at all.
Six free months of two more Canopy products: Concise, our quality measures platform, and Radia 360, our patient engagement platform. Radia 360's free months include unlimited email campaigns — texting can be added any time during the trial.
